This form is to be completed only after consultation with CWNet to confirm the transfer. Submission of this form does not guarantee transfer of service.
All existing account balances must be finalised prior to transfer. The new account holder must agree to CWNet’s terms, conditions, and service requirements.
Any installed equipment remains the property of CWNet unless otherwise advised and must not be removed, relocated, or tampered with.
CWNet reserves the right to approve or decline any transfer request based on serviceability, account status, or technical requirements.
